Što ako jednostavna tableta može pomoći u izlječenju od covida? Američki farmakološki giganti Merck i Pfizer nedavno su objavili ohrabrujuće rezultate ispitivanja svojih oralnih lijekova za tu pandemijsku bolest. 

Radi se o tabletama koje se uzimaju oralno čim se pojave prvi simptomi covida da bi se izbjegli teži oblici bolesti, a time i bolničko liječenje. 

Za takvom vrstom liječenja traga se otkako je počela ova globalna zdravstvena kriza. 

Nakon višemjesečnih istraživanja Merck i Pfizer objavili su da su stigli do cilja. 

Merck je početkom listopada objavio da za svoju tabletu molnupiravir traži dopuštenje za upotrebu u SAD-u, a Pfizer je isto učinio prošli tjedan za svoj paxlovid.

U oba slučaja riječ je o antiviralnim lijekovima koji djeluju tako što smanjuju mogućnost repliciranja virusa, čime se usporava razvoj bolesti. 

Obje kompanije tvrde da klinička ispitivanja pokazuju da korištenje lijekova značajno smanjuje rizik od hospitalizacije. 

Tvrtke tvrde da molnupiravir rizik smanjuje za 50 posto, a paxlovid za 89 posto, premda se stope učinkovitosti ne bi trebale izravno uspoređivati zbog drugačijih protokola za provedbu ispitivanja. 

Antidepresiv koji je već dostupan javnosti, fluvoksamin, također je pokazao ohrabrujuće rezultate u sprečavanju ozbiljnih oblika bolesti, pokazala je studija brazilskih znanstvenika objavljena u listopadu u Lancetu. 

Zašto su važne?

Ako se učinkovitost tableta potvrdi, bit će to velik korak naprijed u borbi s covidom i zajedno s cjepivom pojačat će terapijski arsenal protiv virusa. 

Terapije već postoje, uglavnom u obliku sintetskih antitijela, no ti se lijekovi ubrizgavaju i obično se daju pacijentima koji su već teško bolesni.

S druge strane, tablete se mogu brzo prepisati te ih se može jednostavno uzimati, a da pacijent ne napušta dom. 

Liječenje Merckovim i Pfizerovim tabletama, koje zasad ne izazivaju puno nuspojava, trajalo bi pet dana po dvije doze dnevno. 

"Uspjeh tih anitviralnih lijekova otvara novo poglavlje u sprečavanju teških posljedica covida", rekao je britanski virolog Stephen Griffin. 

Ograničenja? 

Teško je procijeniti uspjeh Merckovih i Pfizerovih terapija jer još nisu objavili podatke o kliničkim ispitivanjima. 

Francuska stručnjakinja Karine Lacombe upozorila je da takve objave farmaceutskih kompanija treba uzeti s oprezom sve dok njihova ispitivanja ne prođu znanstvene recenzije. 

Istaknula je i da bi im takve tablete mogle donijeti "potencijalno golemo" tržište. 

Neovisni stručnjaci kažu da neki elementi ukazuju na to da Merck i Pfizer ne daju lažna obećanja. Obje su kompanije klinička istraživanja prekinule ranije nego što se očekivalo uz blagoslov neovisnih nadzornih odbora, jer su rezultati bili jako dobri. 

Kad je antidepresiv fluvoksamin posrijedi, podaci su dostupni i naišli su na neke prigovore. 

Nekoliko znanstvenika prigovorilo je autorima studije da se nisu ograničili samo na procjenu učestalosti hospitalizacija nego su analizirali i učestalost produljenog boravka na liječenju u bolnicama, što, po njihovu mišljenju komplicira interpretaciju podataka. 

Kada i koliko? 

Merckov molnupiravir već je odobren u Britaniji. Tamošnje zdravstvene vlasti dale su mu zeleno svjetlo za liječenje osoba podložnih većem riziku od razvoja teške kliničke slike poput starijih, pretilih i dijabetičara. 

Američke i europske vlasti po ubrzanoj proceduri evaluiraju lijekove. 

Nekoliko zemalja već je naručilo zalihe molnupiravira, poput Sjedinjenih Država koje su naručile 1,7 milijuna doza. 

Američka narudžba daje naslutiti i koliko bi lijek mogao stajati. Ukupna joj je vrijednost 1,2 milijarde dolara, što bi značilo da petodnevna terapija molnupiravirom košta oko 700 dolara. 

Pfizer još nije objavio cijenu paxlovida, ali je obećao da će biti "pristupačna".

Što ako jednostavna tableta može pomoći u izlječenju od covida? Američki farmakološki giganti Merck i Pfizer nedavno su objavili ohrabrujuće rezultate ispitivanja svojih oralnih lijekova za tu pandemijsku bolest. 

Radi se o tabletama koje se uzimaju oralno čim se pojave prvi simptomi covida da bi se izbjegli teži oblici bolesti, a time i bolničko liječenje. 

Za takvom vrstom liječenja traga se otkako je počela ova globalna zdravstvena kriza. 

Nakon višemjesečnih istraživanja Merck i Pfizer objavili su da su stigli do cilja. 

Merck je početkom listopada objavio da za svoju tabletu molnupiravir traži dopuštenje za upotrebu u SAD-u, a Pfizer je isto učinio prošli tjedan za svoj paxlovid.

U oba slučaja riječ je o antiviralnim lijekovima koji djeluju tako što smanjuju mogućnost repliciranja virusa, čime se usporava razvoj bolesti. 

Obje kompanije tvrde da klinička ispitivanja pokazuju da korištenje lijekova značajno smanjuje rizik od hospitalizacije. 

Tvrtke tvrde da molnupiravir rizik smanjuje za 50 posto, a paxlovid za 89 posto, premda se stope učinkovitosti ne bi trebale izravno uspoređivati zbog drugačijih protokola za provedbu ispitivanja. 

Antidepresiv koji je već dostupan javnosti, fluvoksamin, također je pokazao ohrabrujuće rezultate u sprečavanju ozbiljnih oblika bolesti, pokazala je studija brazilskih znanstvenika objavljena u listopadu u Lancetu. 

Zašto su važne?

Ako se učinkovitost tableta potvrdi, bit će to velik korak naprijed u borbi s covidom i zajedno s cjepivom pojačat će terapijski arsenal protiv virusa. 

Terapije već postoje, uglavnom u obliku sintetskih antitijela, no ti se lijekovi ubrizgavaju i obično se daju pacijentima koji su već teško bolesni.

S druge strane, tablete se mogu brzo prepisati te ih se može jednostavno uzimati, a da pacijent ne napušta dom. 

Liječenje Merckovim i Pfizerovim tabletama, koje zasad ne izazivaju puno nuspojava, trajalo bi pet dana po dvije doze dnevno. 

"Uspjeh tih anitviralnih lijekova otvara novo poglavlje u sprečavanju teških posljedica covida", rekao je britanski virolog Stephen Griffin. 

Ograničenja? 

Teško je procijeniti uspjeh Merckovih i Pfizerovih terapija jer još nisu objavili podatke o kliničkim ispitivanjima. 

Francuska stručnjakinja Karine Lacombe upozorila je da takve objave farmaceutskih kompanija treba uzeti s oprezom sve dok njihova ispitivanja ne prođu znanstvene recenzije. 

Istaknula je i da bi im takve tablete mogle donijeti "potencijalno golemo" tržište. 

Neovisni stručnjaci kažu da neki elementi ukazuju na to da Merck i Pfizer ne daju lažna obećanja. Obje su kompanije klinička istraživanja prekinule ranije nego što se očekivalo uz blagoslov neovisnih nadzornih odbora, jer su rezultati bili jako dobri. 

Kad je antidepresiv fluvoksamin posrijedi, podaci su dostupni i naišli su na neke prigovore. 

Nekoliko znanstvenika prigovorilo je autorima studije da se nisu ograničili samo na procjenu učestalosti hospitalizacija nego su analizirali i učestalost produljenog boravka na liječenju u bolnicama, što, po njihovu mišljenju komplicira interpretaciju podataka. 

Kada i koliko? 

Merckov molnupiravir već je odobren u Britaniji. Tamošnje zdravstvene vlasti dale su mu zeleno svjetlo za liječenje osoba podložnih većem riziku od razvoja teške kliničke slike poput starijih, pretilih i dijabetičara. 

Američke i europske vlasti po ubrzanoj proceduri evaluiraju lijekove. 

Nekoliko zemalja već je naručilo zalihe molnupiravira, poput Sjedinjenih Država koje su naručile 1,7 milijuna doza. 

Američka narudžba daje naslutiti i koliko bi lijek mogao stajati. Ukupna joj je vrijednost 1,2 milijarde dolara, što bi značilo da petodnevna terapija molnupiravirom košta oko 700 dolara. 

Pfizer još nije objavio cijenu paxlovida, ali je obećao da će biti "pristupačna".
