Takozvana histotripsija je postupak kod kojeg se ultrazvučni valovi ciljano usmjeravaju na tumor jetre. To traje samo nekoliko milisekundi, ali je krajnje intenzivno. Cilj su mikromjehurići koji se nalaze u svakom tkivu, pa i u tkivu tumora. Kada ih pogodi ultrazvučni val, oni se povećavaju. A kada dostignu određeni volumen, pucaju i uništavaju stanice tumora. Nakon toga ostaje tekuća masa koju imunosni sustav otprema tijekom idućih tjedana.

Liječnici mogu uživo promatrati ponašanje mjehurića koristeći dijagnostički ultrazvuk kakav se primjenjuje kod pregleda trudnica. Primjena histotripsije još je uvijek ograničena samo na rak jetre, piše Gudrun Heise za Deutsche Welle.

Ovom metodom ne uništava se zdravo tkivo jer je ultrazvuk snažno fokusiran na tumor. Nisu više potrebni operacija, kemoterapija ili zračenje. Poznato je da te metode liječenja jako opterećuju tijelo. Recimo, zračenje djeluje na sve što se nalazi u tijelu na putu do tumora, pa dakle i na zdravo tkivo.

Manje nuspojave i kraći oporavak

Ovaj je postupak razvijen na američkom Sveučilištu Michigan u okviru kliničke studije #HOPE4LIVER ("Nada za jetru"). Na tamošnjem centru Rogel Cancer Center od 2021. provode se studije na ljudima. To je ugledni centar za liječenje oboljelih od tumora. Na tom međunarodnom projektu surađivala su i dva sveučilišta iz Njemačke: Gradska klinika Braunschweig i Sveučilišna klinika Magdeburg.

Tijekom te studije tretirani su pacijentice i pacijenti s primarnim i metastaziranim tumorima jetre, kod kojih su već bile iscrpljene sve mogućnosti konvencionalne terapije.

Rak jetre jedan je od deset vrsta zloćudnih tumora kod kojih su prognoze za liječenje loše i šanse za preživljavanje male. Samo oko 20 posto oboljelih nakon dijagnoze živi još pet godina.

Za pacijentice i pacijente histotripsija ne znači samo da ih se može liječiti bez opterećenja kao što su zračenje i kemoterapija. Ova nova metoda znači i manje problema s nuspojavama lijekova, a i oporavak je daleko kraći nego nakon operacije. Kako pokazuje studija, liječenje novom metodom je sve u svemu manje naporno za oboljeloga, ali je krajnje učinkovito.

Primjena i na rak bubrega ili prostate

Čini se da je histotripsija učinkovita i kod metastaza. U prvim studijama na štakorima imunosni sustav je uz pomoć histotripsije nakon uništenja 50 do 75 posto tumora jetre mogao ukloniti i ostatak stanica zahvaćenih rakom. Kod više od 80 posto životinja na kojima je testirana ta metoda nije bilo znakova mogućih metastaza.

No histotripsija ne otvara nove mogućnosti samo kod uništavanja tumora. Dvije predkliničke studije provedene prošle godine na glodavcima pokazale su da imunosni sustav među ostalim uči i kako prepoznati stanice raka kao prijetnju i aktivirati prirodni imunosni odgovor na rak.

Histotripsija se još uvijek nalazi u ranom stadiju kliničke primjene. U Njemačkoj je za sada dopuštena samo u okviru znanstvenih studija. No znanstvenici polaze od toga da bi to mogla biti vrlo dobra neinvanzivna metoda liječenja oboljelih od raka jetre.

A nadaju se i da će se histotripsijarazviti u terapiju koja će se moći primjenjivati kod svih tumora u cijelom tijelu, a ne samo na jetri, pa polaze od toga da će se tako ubuduće liječiti i rak bubrega ili prostate, piše Deutsche Welle.

Takozvana histotripsija je postupak kod kojeg se ultrazvučni valovi ciljano usmjeravaju na tumor jetre. To traje samo nekoliko milisekundi, ali je krajnje intenzivno. Cilj su mikromjehurići koji se nalaze u svakom tkivu, pa i u tkivu tumora. Kada ih pogodi ultrazvučni val, oni se povećavaju. A kada dostignu određeni volumen, pucaju i uništavaju stanice tumora. Nakon toga ostaje tekuća masa koju imunosni sustav otprema tijekom idućih tjedana.

Liječnici mogu uživo promatrati ponašanje mjehurića koristeći dijagnostički ultrazvuk kakav se primjenjuje kod pregleda trudnica. Primjena histotripsije još je uvijek ograničena samo na rak jetre, piše Gudrun Heise za Deutsche Welle.

Ovom metodom ne uništava se zdravo tkivo jer je ultrazvuk snažno fokusiran na tumor. Nisu više potrebni operacija, kemoterapija ili zračenje. Poznato je da te metode liječenja jako opterećuju tijelo. Recimo, zračenje djeluje na sve što se nalazi u tijelu na putu do tumora, pa dakle i na zdravo tkivo.

Manje nuspojave i kraći oporavak

Ovaj je postupak razvijen na američkom Sveučilištu Michigan u okviru kliničke studije #HOPE4LIVER ("Nada za jetru"). Na tamošnjem centru Rogel Cancer Center od 2021. provode se studije na ljudima. To je ugledni centar za liječenje oboljelih od tumora. Na tom međunarodnom projektu surađivala su i dva sveučilišta iz Njemačke: Gradska klinika Braunschweig i Sveučilišna klinika Magdeburg.

Tijekom te studije tretirani su pacijentice i pacijenti s primarnim i metastaziranim tumorima jetre, kod kojih su već bile iscrpljene sve mogućnosti konvencionalne terapije.

Rak jetre jedan je od deset vrsta zloćudnih tumora kod kojih su prognoze za liječenje loše i šanse za preživljavanje male. Samo oko 20 posto oboljelih nakon dijagnoze živi još pet godina.

Za pacijentice i pacijente histotripsija ne znači samo da ih se može liječiti bez opterećenja kao što su zračenje i kemoterapija. Ova nova metoda znači i manje problema s nuspojavama lijekova, a i oporavak je daleko kraći nego nakon operacije. Kako pokazuje studija, liječenje novom metodom je sve u svemu manje naporno za oboljeloga, ali je krajnje učinkovito.

Primjena i na rak bubrega ili prostate

Čini se da je histotripsija učinkovita i kod metastaza. U prvim studijama na štakorima imunosni sustav je uz pomoć histotripsije nakon uništenja 50 do 75 posto tumora jetre mogao ukloniti i ostatak stanica zahvaćenih rakom. Kod više od 80 posto životinja na kojima je testirana ta metoda nije bilo znakova mogućih metastaza.

No histotripsija ne otvara nove mogućnosti samo kod uništavanja tumora. Dvije predkliničke studije provedene prošle godine na glodavcima pokazale su da imunosni sustav među ostalim uči i kako prepoznati stanice raka kao prijetnju i aktivirati prirodni imunosni odgovor na rak.

Histotripsija se još uvijek nalazi u ranom stadiju kliničke primjene. U Njemačkoj je za sada dopuštena samo u okviru znanstvenih studija. No znanstvenici polaze od toga da bi to mogla biti vrlo dobra neinvanzivna metoda liječenja oboljelih od raka jetre.

A nadaju se i da će se histotripsijarazviti u terapiju koja će se moći primjenjivati kod svih tumora u cijelom tijelu, a ne samo na jetri, pa polaze od toga da će se tako ubuduće liječiti i rak bubrega ili prostate, piše Deutsche Welle.
