Na Zavodu za kardiokirurgiju KBC-a Split uskoro se planira prva ugradnja umjetnog srca, najavio je Hini pročelnik Klinike za kardiokirurgiju te bolnice Mate Petričević.

Operacija ugradnje dugotrajne mehaničke cirkulacijske potpore, tj. umjetnog srca, traje otprilike osam sati, a prvi operacijski zahvat u Splitu obavit će se na Zavodu za Kardiokirurgiju KBC-a Split.

Ta vrsta operacijskog zahvata u Hrvatskoj se izvodi još u KBC-u Zagreb, gdje je formiran tim za mehaničku cirkulacijsku potporu čiji je član donedavno bio i Petričević.

Riječ je o kompleksnoj proceduri koja uključuje visokospecijalizirani tim koji sačinjavaju kirurg, anesteziolog, kardiolog i medicinske sestre u Jedinici intenzivnog liječenja i na odjelu, te fizioterapeuti i psiholozi. 

"Bolnice u kojima se provodi ugradnja umjetnog srca u statusu su centara izvrsnosti na području kardijalne medicine.

Donedavno su te vrste operacija bile rezervirane za svega nekoliko centara u SAD-u i u Europi, a uskoro bi se i KBC Split trebao naći na svjetskoj karti ekspertnih centara za ugradnju umjetnog srca", kaže Petričević.

Kandidati za ugradnju umjetnog srca mahom su bolesnici kod kojih je srce u fazi terminalnog zatajenja, odnosno funkcija srca im je dramatično niska, a konzervativne metode liječenja su iscrpljene. 

Nažalost, sve je više mladih bolesnika kojima je to potrebno. Njima umjetno srce može poslužiti kao premoštenje do transplantacije srca, dok je kod starijih ljudi umjetno srce konačna forma liječenja bez potrebe za kasnijom transplantacijom.

"U posljednje vrijeme svjedočimo bolesnicima kojima je ugrađeno umjetno srce kao terapija premoštenja do transplantacije, ali oni zadovoljni umjetnim srcem kasnije odustaju od transplantacije", rekao je Petričević.

Pacijent uči živjeti s umjetnim srcem

Nakon operacije pacijenti u bolnici ostaju tri tjedna jer je edukacija bolesnika posebno važan aspekt liječenja. Pacijent u bolnici uči živjeti s uređajem kako bi nakon izlaska bio samostalan. 

Komplikacije najčešće nastaju kada se uzima previše lijekova za razrjeđenje krvi pa dolazi do krvarenja ili se uzme premala doza pa ugrušak uđe u sistem crpke. U tom slučaju ugrušak se treba otopiti ili, u najgorem slučaju, zamijeniti uređaj novim. 

U tijelu pumpe je propeler koji levitira u magnetskom polju i vrti se do deset tisuća okretaja u minuti. Vrteći se stvara protok krvi po principu Arhimedovog vijka, a taj protok je konstantan, zbog čega bolesnici nakon ugradnje umjetnog srca nemaju više mjerljiv puls. 

"Prijašnje generacije uređaja generirale su pulsni protok, međutim te vrste uređaja bile su veće i kompleksnije i stoga podložnije kvarovima. Današnja tehnologija je dosegla visoku razinu, a kroz nekoliko godina očekujem da će ta vrsta uređaja imati dimenzije baterija za kućansku upotrebu", kaže Petričević.

Ako operacija prođe u redu, a terapija se provodi bez ikakvih komplikacija ugrađeni uređaj može trajati i više od 20 godina. 

Trenutno se provode istraživanja i sve se precizno dokumentira. Rezultati su ohrabrujući, a Petričević vjeruje da će s vremenom biti sve bolji. 

"Kada uzmete u obzir 10 ili 20 godina života s umjetnim srcem i to usporedite sa stanjem terminalnog srčanog bolesnika kod kojega je iscrpljena mogućnost liječenja, jasno je da se radi o velikom koraku u liječenju srčanih bolesti", kaže.

Na Zavodu za kardiokirurgiju KBC-a Split uskoro se planira prva ugradnja umjetnog srca, najavio je Hini pročelnik Klinike za kardiokirurgiju te bolnice Mate Petričević.

Operacija ugradnje dugotrajne mehaničke cirkulacijske potpore, tj. umjetnog srca, traje otprilike osam sati, a prvi operacijski zahvat u Splitu obavit će se na Zavodu za Kardiokirurgiju KBC-a Split.

Ta vrsta operacijskog zahvata u Hrvatskoj se izvodi još u KBC-u Zagreb, gdje je formiran tim za mehaničku cirkulacijsku potporu čiji je član donedavno bio i Petričević.

Riječ je o kompleksnoj proceduri koja uključuje visokospecijalizirani tim koji sačinjavaju kirurg, anesteziolog, kardiolog i medicinske sestre u Jedinici intenzivnog liječenja i na odjelu, te fizioterapeuti i psiholozi. 

"Bolnice u kojima se provodi ugradnja umjetnog srca u statusu su centara izvrsnosti na području kardijalne medicine.

Donedavno su te vrste operacija bile rezervirane za svega nekoliko centara u SAD-u i u Europi, a uskoro bi se i KBC Split trebao naći na svjetskoj karti ekspertnih centara za ugradnju umjetnog srca", kaže Petričević.

Kandidati za ugradnju umjetnog srca mahom su bolesnici kod kojih je srce u fazi terminalnog zatajenja, odnosno funkcija srca im je dramatično niska, a konzervativne metode liječenja su iscrpljene. 

Nažalost, sve je više mladih bolesnika kojima je to potrebno. Njima umjetno srce može poslužiti kao premoštenje do transplantacije srca, dok je kod starijih ljudi umjetno srce konačna forma liječenja bez potrebe za kasnijom transplantacijom.

"U posljednje vrijeme svjedočimo bolesnicima kojima je ugrađeno umjetno srce kao terapija premoštenja do transplantacije, ali oni zadovoljni umjetnim srcem kasnije odustaju od transplantacije", rekao je Petričević.

Nakon operacije pacijenti u bolnici ostaju tri tjedna jer je edukacija bolesnika posebno važan aspekt liječenja. Pacijent u bolnici uči živjeti s uređajem kako bi nakon izlaska bio samostalan. 

Komplikacije najčešće nastaju kada se uzima previše lijekova za razrjeđenje krvi pa dolazi do krvarenja ili se uzme premala doza pa ugrušak uđe u sistem crpke. U tom slučaju ugrušak se treba otopiti ili, u najgorem slučaju, zamijeniti uređaj novim. 

U tijelu pumpe je propeler koji levitira u magnetskom polju i vrti se do deset tisuća okretaja u minuti. Vrteći se stvara protok krvi po principu Arhimedovog vijka, a taj protok je konstantan, zbog čega bolesnici nakon ugradnje umjetnog srca nemaju više mjerljiv puls. 

"Prijašnje generacije uređaja generirale su pulsni protok, međutim te vrste uređaja bile su veće i kompleksnije i stoga podložnije kvarovima. Današnja tehnologija je dosegla visoku razinu, a kroz nekoliko godina očekujem da će ta vrsta uređaja imati dimenzije baterija za kućansku upotrebu", kaže Petričević.

Ako operacija prođe u redu, a terapija se provodi bez ikakvih komplikacija ugrađeni uređaj može trajati i više od 20 godina. 

Trenutno se provode istraživanja i sve se precizno dokumentira. Rezultati su ohrabrujući, a Petričević vjeruje da će s vremenom biti sve bolji. 

"Kada uzmete u obzir 10 ili 20 godina života s umjetnim srcem i to usporedite sa stanjem terminalnog srčanog bolesnika kod kojega je iscrpljena mogućnost liječenja, jasno je da se radi o velikom koraku u liječenju srčanih bolesti", kaže.

Na Zavodu za kardiokirurgiju KBC-a Split uskoro se planira prva ugradnja umjetnog srca, najavio je Hini pročelnik Klinike za kardiokirurgiju te bolnice Mate Petričević.

Operacija ugradnje dugotrajne mehaničke cirkulacijske potpore, tj. umjetnog srca, traje otprilike osam sati, a prvi operacijski zahvat u Splitu obavit će se na Zavodu za Kardiokirurgiju KBC-a Split.

Ta vrsta operacijskog zahvata u Hrvatskoj se izvodi još u KBC-u Zagreb, gdje je formiran tim za mehaničku cirkulacijsku potporu čiji je član donedavno bio i Petričević.

Riječ je o kompleksnoj proceduri koja uključuje visokospecijalizirani tim koji sačinjavaju kirurg, anesteziolog, kardiolog i medicinske sestre u Jedinici intenzivnog liječenja i na odjelu, te fizioterapeuti i psiholozi.
