Ljekarnici će ubuduće u Hrvatskoj educirati kronične pacijente o pravilnom uzimanju terapije, nuspojavama i poboljšanju kvalitete života, u sklopu projekta, vrijednog više od pola milijuna eura, koji pokreće Ministarstvo zdravstva.
Plan je uključiti ljekarnike u praćenje ishoda liječenja jer nacionalna i svjetska istraživanja pokazuju da gotovo 50 posto pacijenata ne uzima propisane lijekove ili ih ne uzimaju na adekvatan način, kažu u Ministarstvu.
Rezultat je propadanje gotovo polovine lijekova propisanih na recept, ali i pojava komplikacija u liječenju, a "najneuredniji" su kronični pacijenti s dijabetesom, astmom i kardiovaskularnim bolestima.
U Ministarstvu kažu da je u tijeku postupak javne nabave za novi informatički sustav za koji su spremni izdvojiti 570 tisuća eura uključujući PDV. Bit će povezan s Centralnim zdravstvenim informacijskim sustavom, tako da će podaci upisani u ljekarnama biti vidljivi i u liječničkim ordinacijama.
Ljekarnici će ubuduće provoditi edukaciju o bolesti, lijeku, pravilnom uzimanju terapije, prepoznavanju i saniranju nuspojava te edukaciju o nefarmakološkim mjerama radi unaprjeđenja kvalitete života kroničnih pacijenata.
U konačnici to bi trebalo smanjiti stopu odustajanja od terapije, te omogućiti bolju kontrolu bolesti kako bi se spriječilo dodatno liječenja komplikacija.
Projekt uvođenja sustava praćenja ishoda liječenja izvanbolničkih pacijenata s naglaskom na kronične pacijente u javnim ljekarnama dio je Nacionalnog plana za oporavak i otpornost sa završnim rokom za realizaciju do kraja 2025. godine.
Predsjednica Hrvatske ljekarničke komore Ana Soldo smatra da nove zadaće neće oduzimati previše vremena ljekarnicima, ako informatički sustav bude dobro napravljen. Napominje da ljekarnici i sada uvode pacijenta u protokol "prvog uzimanja lijeka", no sada će pratiti dinamiku uzimanja terapije, što će sve biti dostupno i vidljivo liječnicima u primarnoj zdravstvenoj zaštiti.
Liječnici obiteljske medicine dobivat će informacije o suradljivosti pacijenta te o mogućim problemima vezanim uz farmakoterapiju.
U Ministarstvu kažu da je ovaj projekt još jedan u nizu za poboljšanje zdravstevene pismenosti građana, uz preventivne zdravstvene preglede, poticanje zdravih životnih navika te reviziju postojećih i uvođenjem novih Nacionalnih preventivnih programa.

Ljekarnici će ubuduće u Hrvatskoj educirati kronične pacijente o pravilnom uzimanju terapije, nuspojavama i poboljšanju kvalitete života, u sklopu projekta, vrijednog više od pola milijuna eura, koji pokreće Ministarstvo zdravstva.
Plan je uključiti ljekarnike u praćenje ishoda liječenja jer nacionalna i svjetska istraživanja pokazuju da gotovo 50 posto pacijenata ne uzima propisane lijekove ili ih ne uzimaju na adekvatan način, kažu u Ministarstvu.
Rezultat je propadanje gotovo polovine lijekova propisanih na recept, ali i pojava komplikacija u liječenju, a "najneuredniji" su kronični pacijenti s dijabetesom, astmom i kardiovaskularnim bolestima.
U Ministarstvu kažu da je u tijeku postupak javne nabave za novi informatički sustav za koji su spremni izdvojiti 570 tisuća eura uključujući PDV. Bit će povezan s Centralnim zdravstvenim informacijskim sustavom, tako da će podaci upisani u ljekarnama biti vidljivi i u liječničkim ordinacijama.
Ljekarnici će ubuduće provoditi edukaciju o bolesti, lijeku, pravilnom uzimanju terapije, prepoznavanju i saniranju nuspojava te edukaciju o nefarmakološkim mjerama radi unaprjeđenja kvalitete života kroničnih pacijenata.
U konačnici to bi trebalo smanjiti stopu odustajanja od terapije, te omogućiti bolju kontrolu bolesti kako bi se spriječilo dodatno liječenja komplikacija.
Projekt uvođenja sustava praćenja ishoda liječenja izvanbolničkih pacijenata s naglaskom na kronične pacijente u javnim ljekarnama dio je Nacionalnog plana za oporavak i otpornost sa završnim rokom za realizaciju do kraja 2025. godine.
Predsjednica Hrvatske ljekarničke komore Ana Soldo smatra da nove zadaće neće oduzimati previše vremena ljekarnicima, ako informatički sustav bude dobro napravljen. Napominje da ljekarnici i sada uvode pacijenta u protokol "prvog uzimanja lijeka", no sada će pratiti dinamiku uzimanja terapije, što će sve biti dostupno i vidljivo liječnicima u primarnoj zdravstvenoj zaštiti.
Liječnici obiteljske medicine dobivat će informacije o suradljivosti pacijenta te o mogućim problemima vezanim uz farmakoterapiju.
U Ministarstvu kažu da je ovaj projekt još jedan u nizu za poboljšanje zdravstevene pismenosti građana, uz preventivne zdravstvene preglede, poticanje zdravih životnih navika te reviziju postojećih i uvođenjem novih Nacionalnih preventivnih programa.
